Abstract

Introduction: The carriage of carbapenemase-producing bacilli (CPB) in hospitalized patients implies active surveillance to contain their spread, given the few therapeutic options for infections caused by these microorganisms. Objective: To determine the carriage of CPBs in patients hospitalized in the Adult Intensive Care Unit (ICU) of the Hospital Interzonal de Agudos “Evita” in Lanús, Buenos Aires, Argentina. Materials and Methods: A descriptive, prospective, weekly study was carried out for 4 months. Rectal swabs were taken from the patients according to established criteria. The samples were studied by realtime multiplex PCR (BD MaxTM System, Becton Dickinson, USA), with detection of the following enzymes: KPC, NDM, VIM/IMP and OXA. The information was collected in a form, analyzed in Excel sheets, and Xi 2 was used to determine the significant differences (p<0.05; Epidat 3.0 program). Results: A total of 99 swabs were obtained from 74 patients; 36 were positive, representing a CPB carriage of 48.6%. The percentage distribution of the different carbapenemases was: 40.4% NDM; 30.7% OXA; 17.3% KPC; 5.8% MIV; 5.8% VIM/ IMP. In 38.9% of carriers, colonization by more than one type of carbapenemase was demonstrated. The subsequent appearance of a focus of infection by a CPB was higher in colonized patients (p<0.05). Conclusions: These data result from the first active surveillance study of patients colonized by CPBs, using real-time PCR. Carrying was 48.6% and constitutes a baseline to monitor the control actions implemented.
